Consent Form for Collection, Use, and Provision of Personal Information

Asia Hope Camp Organization and Korea Plaza Hiroba Co., Ltd. (hereinafter referred to as “the Organization”) collect and use personal information in accordance with the Personal Information Protection Act. Please read the following information carefully before deciding whether to give your consent.

All personal information you provide will be used only for the purposes stated below and will not be used for any other purposes.

1. Items to Be Collected and Purpose of Use

a. Required Items:

  • Name, date of birth (age), nationality, residential area, address, contact information (mobile phone), photo, email, educational background, self-reported health status

  • For homestay host families: photos of the home (living room, guest room, family), home address, nearest subway station

  • For trainee courses: body measurements (height, weight, dental correction), dance video, vocal video

b. Purpose of Collection and Use:

  • Operation and management of the program

  • Screening, audition processes, and qualification verification (for trainees)

  • Accident prevention and participant safety

  • Personal information may be provided to affiliated academies or entertainment planners for external training or audition arrangements

2. Retention and Use Period

  • From the date of consent until the stated purposes are fulfilled

  • However, reports and reflections submitted after participating in the program may be uploaded to the Organization's website and retained for up to 30 years for promotional or recordkeeping purposes and will not be deleted

3. Right to Refuse Consent

  • You have the right to refuse consent to the collection and use of personal information

  • However, refusal to consent may result in restrictions or disadvantages, such as limitations on program application or selection
